Each card you wish to liquidate will require an authorization. We've provided one form that will work for three cards.

If you need use more than three cards, after this form is submitted, you'll be allowed to fill out the form again for the additional cards.

CREDIT CARD DISCLAIMER

When you use a high majority of your credit card's limit shortly after acquiring the card, this creates a risk alert with your credit card company. 

We try to help credit card users avoid this risk by encouraging users to only use the percentage we suggest, or lower, to minimize a usage alert with your credit card issuer.

Recently CHASE credit card has become more vigilant about the monitoring of their clients' usage risks. Using our service to pull the majority of the credit line from your CHASE credit card may trigger an alert and the card is shut down. CHASE does not allow you to appeal this decision. It does impact your credit score and usage ratings.

We do not recommend using a CHASE credit card for our services. If you decide to use a CHASE card, we recommend using less than half of your credit limit on the specific CHASE card. We have no control over the CHASE risk assessment process. We are letting customers know based upon our recent awareness of the change in the CHASE risk management policies.

If you choose to proceed with a CHASE transaction, you understand that the law office is not liable if your card is shut down, or how the transaction impacts your card, credit report, or other financial consequences.

If you would like the same card used in multiple transactions, you can add the amount of the transactions as follows: 1st - $4795, 2nd - $9565, 3rd - $9975, 4th - 7565, etc., on the same line
