CREDIT CARD AUTHORIZATION

Each card you wish to liquidate will require an authorization. We've provided one form that will work for three cards.

If you need use more than three cards, after this form is submitted, you'll be allowed to fill out the form again for the additional cards.

If you would like the same card used in multiple transactions, you can add the amount of the transactions as follows: 1st - $4795, 2nd - $9565, 3rd - $9975, 4th - 7565, etc., on the same line To help limit risk assesment with your credit card company:

• Per Transactions should not be more than $10k.

• Transactions should not be round numbers like $500

• No duplicate amounts, meaning don't run the exact same number twice on the same card.

• If you have a $10,000 amount you want to run, break it into two transaction like $5,940 and $4,060

• If you want to run one transaction close to $10,000, use something like $9,975
